Our team is dedicated to providing the highest quality care to patients with highly complex health needs.

We have family physicians, NPs, RNs, Social Work, Private Psychiatry Consult Liaison and we collaborate with various PCN services such as Pharmacy.

We exclusively accept patients on the Provincial Government's Health Connect Registry who are flagged for the PCN Priority Patient Program, and exceptional cases from Acute Care and other organizations.
